The aim of the Register is to record the key achievements of library and information professionals who are CILIP members, so that their contribution to the Profession is not lost upon their retirement or death.

If you wish to be part of the Register, please download the two-page questionnaire which will form the basis of your entry. Due to the requirements of the Data Protection Act, a signed paper copy of your completed questionnaire is required.

If you feel that the two-page questionnaire is too limited, please include a copy of your CV or other relevant information with the returned questionnaire. If you wish, an enhanced questionnaire and full CV can be submitted electronically alongside the paper submission.

Please return the paper questionnaire and any additional material to:
‘Information Team – PAR’, CILIP, Freepost (Licence no WC3122), London WC1B 7BR. Submission of material by e-mail can be made to info@cilip.org.uk

For the first two years the Register was  sponsored by the Retired Members Guild. CILIP remains indebted to the Guild for the work that was put into putting the Register in place. A presentation on the creation of the Register was given at Umbrella 2003 (http://www.umbrella2003.org.uk/speakers/chapmanp.html).
