Forms for Chartership candidates

Remember you must be in CILIP Membership in order to register to Charter

• Registration form
  
  
• Mentor request form Candidates may use this when approaching prospective mentors. (See the mentor pages first).
  
  
• Mentor agreement form 
  
  
• Personal Professional Development Plan This should be completed in discussion with your mentor and sent in to CILIP within six months of registering.
  
  
• Audit sheet This is not a required document but can be useful in identifying evidence of learning to include in the portfolio.
  
  
• Assessment form This is the form used by Board Members to assess your application.
  
  
• Mentor completion form (to be included in your portfolio application)
  
  
• Submission form 
  This form should accompany your final application: 
  
  Form for candidates registered under current regulations;
  Form for candidates registered under 2002 regulations submitting under 2002 regulations;
  Form for candidates registered under previous regulations submitting under current regulations.
