Please bear in mind the following things when filling it in:

• Since this is a collaborative sheet, please be careful when you enter the data
• Check that someone has not already filled in data for your library – if there is something there, you can update the note section if you have new information
• If you would like us to contact you about Libraries at Risk, you can add your email to the contributor column
• Please select ‘changes proposed’ under status – Please do not fill in anything else
• Please provide any details you can about the changes under the note column
• The map draws on the data automatically twice a day, which means the information you have entered might not appear immediately, please check back in a few hours
• If you see some information that is out of date or erroneous, you can overwrite what's there, but please make sure to leave a note in the comments to help us understand this change
